Where do you start?

Know any coaches to say these kinds of things?
"straighten your head, pull that right arm in and pull that right knee out, and stop crossing over in your gait, widen your step width"

Yesterday I again discussed arm swing and perhaps why not to coach it out if you merely do not like how it looks in your client. I also mentioned that head tilt, torso/trunk deviations are likely compensations, but it can go both ways. One has to solve for the problem, and not coach the changes we wish to see into the client. Where do you start with a client? Their head tilt? the Right arm abduction? The medially collapsed knee? The abducting swing leg knee ? Where do you start? If you do not examine your client, understand the principles of cause and effect of aberrant human mechanics, you just might recommend an orthotic, a stability shoe, and coach "straighten your head, pull that right arm in and pull that right knee out, and stop crossing over in your gait, widen your step width". That is fine if that is what you choose to do, but i suspect i will also see you at the county fair playing "Whack a Mole". It is the same game. You'll always lose your money, and realize that game never ends, not until you solve for "X" (the cause). -Dr. Allen

And then there is # 245....

Look at the position of his knee with respect to his foot. Both are pointing outward into external rotation. They are in the same plane so a torsion is unlikely, but he is probably trying to gain some stability as his center of gravity moves across his body.

Now look at his pelvis and see how it is tilted (or dropped) down on the left. We would definitely want to check his right gluteus medius and his left quadratus lumborum.

And what about that arm swing? Is he creating more space on the left (ie abducting his left arm) from his body lean to the right? More than likely, this is to make up for his hip muscle problem. we say this because his head tilt is to the left. Remember that your body will always try to make your visual fields parallel with the ground, so as his body goes to the right, his head will go to the left. There must be a great deal of body rotation going on, as his entire jersey has moved to his right!

remember last mondays post? (if not, click here). Here is one way of telling whether your (or someone else’s) vestibular system is working. It will also give you an idea of how some people compensate.

Ready?

  • Stand up (barefoot or shoes does not matter).
  • place your hands resting on the top of your hips with your thumbs to the back (like your Mom used to, when you were in trouble). Your thumbs should be resting on your quadratus lumborum (QL) muscle.
  • tilt your HEAD to the LEFT
  • you should feel the muscle (ie the QL) under your RIGHT thumb contract
  • come back upright


repeat, but this time lean your BODY to the LEFT

  • same thing right? Now check the other side.


Everything OK? Everything fire as it should?

Now lets add another dimension.

  • slide your fingers down so they are just below the crest of the hip, resting above the greater trochanter (the bump on the side of your upper thigh). This should place your fingers on the middle fibers of the gluteus medius.
  • tilt your head (or body ) to the LEFT.
  • You should feel the LEFT gluteus medius and the RIGHT QL contract. These muscles should be paired neurologically. When walking, during stance phase on the LEFT: the LEFT gluteus medius helps to maintain the pelvis level, while the RIGHT QL, assists in hiking the RIGHT side.


If everything works OK, then your vestibulospinal spinal system is intact and your QL and gluteus medius seem to be firing and appropriately paired. If not? That is the subject for another post.
